If your passport has not being uploaded on your profile at the time of registration, please send your recent passport to our care team to complete your profile and ensure access to care. Please Note – the passport must be taken within the past 6 months, showing full face, front view with a plain white or off-white background. Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head.

Simply put, you are not restricted to your default primary care provider (hospital) but can use any of the hospitals eligible for your plan.
